OPTION 1: Full Service NON-IVP Fingerprint Clearance Card - $109.00

• Provide the applicant with a Fingerprint Clearance Card Application
• Capture the applicant’s fingerprints
• Supply the $67.00 payment for AZDPS
• Submit the full packet and payment to AZDPS on the applicant’s behalf

***There is no need to register online with AZDPS ahead of time when choosing this option

OPTION 2: Pre-Registration with AZDPS - $34.00

If you prefer to register directly with the state, please follow these steps:

• Visit psp.azdps.gov and create an account.
• Complete the NON-IVP Fingerprint Clearance Card application online.
• Pay the state fee directly to AZDPS (approximately $68).
• Save or print your payment confirmation receipt—your Registration Number will appear in the bottom-right corner. (will start with P followed by 9 digits)

When you come on site, please bring:

• A valid form of government-issued ID
• Your AZDPS Registration Number

We will capture your fingerprints and you may choose one of the following:

• Self-mailing: We provide you the completed fingerprint card to mail to AZDPS yourself
• Mailing service: We can mail your completed Fingerprint Card for you for an additional $3
